Who builds the repair request — the inspector or the agent?+
The buyer's agent, working from the report link you already sent. There's no account and no login: the same token-secured link that shows the report also carries the builder. You don't have to do anything after delivery for it to be available.
Is this a promulgated or legal form?+
No. It's a negotiation aid generated from your inspection, not a state-promulgated addendum, and it says so on the document. In states that require a specific board form, the agent uses Copy-as-text to move the items into their own addendum. The copy stays editable and carries a plain disclaimer.
Do the inspection photos show up in the request?+
Yes. Photos attached to a finding carry into the repair request, so the request the seller's side receives shows the same evidence documented in the report — not just a line of text.
